To get to our office, turn at the stoplight at West Main and Phoenix and go north two blocks. We are on the corner of Phoenix and C Streets, across from Barron Family Practice.

Our goal is to offer you a wonderful experience when you visit our office, with a caring staff to the finest quality eye care, eyewear and contact lenses.


_______________Office Policy___________________

We are grateful that you have chosen our office for your eye care needs and would like you to know our policy on all matters.

1. Appointment Scheduling:
We will try to meet your needs to schedule routine appointments
as soon as possible. (Emergencies will be worked in same day or,
if Dr.Clark is unavailable, referred to a physician nearby.) When
you call, our receptionist will ask for a phone number or email
address where you can be reached 1-2 days before, to confirm
your appointment. Our policy is that you must confirm that this
time is still good for you and that you will be here at the time
you requested. If your appointment cannot be confirmed, it will
be given to someone on our short-call list and you will be
rescheduled to a time that you can be here.

2. Missed Appointments:
Things happen, and we realize that. Kindly let us know at least
24 hours in advance, if possible and we will do our best to get
your next appointment as soon as possible. Missed confirmed
appointments where we weren't notified will be charged for
that time reserved.

3. Insurance Plans:
We accept many insurance vision and medical plans for your
convenience. Most vision plans cover vision exams for those
with normal seeing problems, but if your exam reveals other
abnormalities, your exam may become a medical visit and a
separate visit may be required to address those issues. Examples
would include someone presenting for a routine exam who
has an eye infection that prevents Dr. Clark from getting an
accurate prescription, someone with diabetes, glaucoma or
cataracts, where a vision exam alone is not appropriate.
Medical visits and testing for medical eye problems will be
charged to your health insurance plan, with deductibles and
copays collected at the day of visit.

4. Contact Lenses:
Contact lenses are medical devices that, worn properly, can
bring excellent vision and confidence to wearers. Our trial
fitting will ensure that you see the best possible with contacts
and that you agree to that, before the prescription is written.
While trial lenses used to determine the best possible vision
are not free, we will go the extra mile to get the appropriate
prescription. Once the determination is finalized and the
contact prescription is written, it is approved for six to twelve
months, depending on the doctor's assessment of the patient's
corneal health, wearing pattern and best interests. Additional
trial lenses requested may be provided for those who comply
with recommended wearing schedule and have purchased
annual supplies through this office. Additional fees apply.
Non-compliant, habitual over-wearers who risk serious
infection or loss of sight will be referred to other eye care
providers.
You must also agree to the wearing schedule appropriate for
your kind of contact lenses, as well, realizing that overwear or
sleeping in contacts can result in serious infection or blindness.
We require annual visits to renew contacts to ensure that
your eyes are healthy and the prescription is correct. You will
be pre-appointed for your visit in one year to keep your
prescription accurate and current, and we charge an assessment
fee to make that determination. Should your new prescription
require a change in the type of lens correction needed, such as
needing astigmatism or bifocal powers, or you do not show for
your annual exam, a refitting fee will be assessed.
Fitting contacts consists of a determination of
the best trials possible, plus options, teaching
insertion and removal techniques, teaching cleaning
and disinfection techniques, follow-up visits to
finalize the prescription.


 

Currently, rebates offered by manufacturers combined with
insurance benefits offered by most vision plans make contact
lenses a very affordable and attractive option!

5. All visits to this office are charged visits, and patients presenting
should expect to pay deductibles or copays at the time of visit.
Emergency visits and workmen's comp injury visits will be
charged, in advance for their treatment, and a fee slip sent to
the appropriate company or insurance agency for our patients.
After hours emergency, foreign body removal or infection
visits require pre-payment of $175, and insurance will then be
billed during normal working hours, so that our patient can then
be reimbursed.

6. Glasses/ Warranties/Adjustments/Remakes:
Our eyewear and contact lens orders will be made in a timely
manner with the emphasis on correct, accurate prescriptions.
While most orders are received in 10 business days, some
require extra time and must pass quality check controls
before being sent to us. These factors make promising an
exact date of delivery untenable, as our most important
goal is getting the product we asked for, without exception.

Our frames and lenses are warranted against breakage for one
year for normal wear and tear. They will not replace frames or
lenses that have been run over with your truck or chewed up
by your dog. Lenses have a scratch-resistant and ultraviolet-
blocking coating, but will scratch if you do not clean them
properly or abuse them. Warranty replacement of scratched lens applies ONLY to separate purchase of Duraguard protection. Our
lab tech will teach you to clean and care for your eyewear.
We offer free adjustments for eyewear purchased from our
dispensary. Adjustments for glasses made elsewhere are $49 initially
and $29 for subsequent visits.

Remaking lenses, for any reason, that are supplied by vision insurance plans are subject to that plans policy for remakes.
If your plan does not have a
remake policy, you will be charged any fee the lab assesses
to remake your glasses, which is also our policy for lenses
not supplied by insurance plan labs.

7. Cancellations / Revisions of Orders:
Cancellations of orders of glasses or contact lenses, or failure
to pick up ordered items within 60 days will result in a
restocking fee of 50%. Modification of custom-ordered glasses
to a different frame than originally ordered:
1. Must be requested within 10 days of dispensing
2. Re-sellable frames will by credited in full
3. Buyer then will be charged for newer frame plus lenses.
Custom-made contact lenses cannot be cancelled once the
order is made.
Every effort will be made to accommodate special requests
or extenuating circumstances in glasses or contact lens
orders but the decision made by our office manager is final.
